What you will do

• Deliver direct care through virtual urgent care and a virtual observation unit, representing approximately 80% of the role.
• Assess urgent-care complaints through video and telephone visits and escalate patients to higher-acuity settings when appropriate.
• Manage acute-on-chronic exacerbations, perform medication reviews and deprescribing, and support optimal clinical outcomes.
• Provide clinical supervision and teaching to remote advanced practice providers, lead clinical rounds, and advise on challenging cases.
• Connect patients with primary care and comprehensive programs including complex care, behavioral health, chronic disease management, and palliative care.
• Promote equitable, whole-person care for vulnerable patient populations.

Requirements

• Board certification and at least 3 years of post-residency experience in Family Medicine, Internal Medicine, Emergency Medicine, or Urgent Care.
• Ability to learn and use electronic health data, medical record, video, telephone, and AI platforms.
• Multi-state licensure is preferred, or willingness to obtain additional licenses.
• Strong clinical judgment, organization, attention to detail, collaboration, and team leadership skills.
• Commitment to care equity, accessibility, whole-person care, and evidence-based practice.

Nice to have

• Telehealth experience.
• Acute-care experience in emergency departments, urgent care, hospital medicine, or similar settings.
• Experience caring for older adults.
• Experience with population health or value-based care programs.
• Spanish fluency.
