Care Manager (Behavioral Health)

Location: Remote in New Jersey, serving members in Essex and Union Counties; candidates must reside in Essex County, Union County, or a nearby bordering county. Up to 50% local travel is required for in-home and site visits. Schedule: Monday–Friday, 9:00 AM–5:00 PM. Closest company office: Iselin, NJ.

Salary: $56,200–$101,000 per year

What you will do

• Assess members’ behavioral health, mental health, substance use disorder, and social determinant needs through phone, digital, in-home, and site visits.
• Develop and maintain personalized care plans for members with high-acuity needs.
• Coordinate access to providers, specialists, community resources, and other required services.
• Monitor treatment side effects, complications, clinical symptoms, care plans, and member outcomes.
• Educate members and families about treatment options, referrals, healthcare benefits, and provider instructions.
• Document care management activities and provide feedback on opportunities to improve care quality and cost-effectiveness.

Requirements

• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
• 2–4 years of related experience.
• Active New Jersey license required.
• Licensed Master’s Behavioral Health Professional, such as LCSW, LMSW, LMFT, LMHC, or LPC, or RN with behavioral health experience, based on state contract requirements.
• For the New Jersey Health Plan: Master’s degree in Behavioral Health or Social Work and an LAC or LSW required.
• Must reside in Essex County, Union County, or a nearby bordering county and be available for up to 50% local travel.
