TL;DR
Supportive Housing Case Manager (Social Services): Providing case management, life skills development, housing support, and referrals for homeless individuals and families with an accent on client assessments, intervention planning, and community outreach. Focus on coordinating public benefits, preventing evictions, connecting clients with medical and mental health services, and tracking outcomes through onsite work and area travel.

Location: Kennewick, Washington, USA. This is an onsite role requiring daily attendance in the office, community outreach, client home visits, and travel throughout the area.

Salary: $21.50–$23.95 per hour

What you will do

  • Conduct client assessments and interviews to determine eligibility and maintain action plans, case notes, and related records.
  • Provide life skills development, targeted interventions, and support to improve independence and prevent eviction.
  • Connect clients with medical, mental health, counseling, housing, and community services.
  • Help clients apply and maintain eligibility for SSI, SSDI, Medicaid, Medicare, SNAP, and other benefits.
  • Conduct home visits, wellness checks, community outreach, client transportation, and area travel as needed.
  • Track outcomes, provide data updates, participate in meetings, and represent Compass at professional and community events.

Requirements

  • Either a bachelor's degree in social services or a related field plus one year of supportive housing or independent living coordination experience, or a high school diploma/GED plus two years of relevant experience.
  • Direct housing experience must be clearly detailed in the resume.
  • Valid driver's license, insurance, and access to a vehicle insured for the applicant are required.
  • Ability to work a flexible schedule, including occasional nights and weekends, and travel throughout the service area.
  • Technology proficiency with Microsoft Office, Zoom, PCs, Android devices, laptops, cell phones, and designated software.
  • Ability to perform CPR/First Aid as needed, lift up to 50 pounds, and be at least 21 years old.
