Collaborative Practice Pharmacist

What you will do

• Provide direct clinical pharmacy care to client employees and improve patient outcomes.
• Collect medication and drug histories, conduct comprehensive medication reviews, monitor therapy, and communicate recommendations to providers.
• Perform medication reconciliation to identify omissions, duplications, dosing errors, interactions, and adherence issues.
• Deliver individual and group education, behavioral counseling, wellness coaching, and preventive care services.
• Work under collaborative practice agreements to assess patients, order medication-related laboratory tests, administer medications, and initiate or adjust drug regimens.
• Serve as the pharmacologic expert for other staff providers and support additional assigned duties.

Requirements

• PharmD or Bachelor of Pharmacy degree.
• Current, unrestricted pharmacist license, including any required state-specific licensure.
• Certified Diabetic Educator certificate, or willingness to obtain it within one year of hire.
• Wellcoaches, ICF-accredited coaching, or equivalent advanced training in motivation and leading change, or willingness to obtain it.
• At least three years of disease state management experience, two years of clinical experience, acceptable board certification, completion of an accredited clinical residency, or demonstrated ability to achieve positive patient outcomes in a similar environment.
• Experience with customer service, clinician and patient education, coaching or mentoring, staff training, and individual or small-group counseling.

Nice to have

• Experience in health promotion or worksite wellness.
• Experience with Microsoft Office, MS Visio, electronic medical records, data tracking, reporting, and workflow or business requirements documentation.
• Knowledge of workplace health and safety concepts and OSHA regulations.
