RN Case Manager (Care Coordination)

Location: Manchester, New Hampshire, United States. Full-time onsite role with an 8:00 a.m.–4:30 a.m. work shift as stated in the posting.

hirify.global’s Care Coordination Department at Elliot Hospital provides care coordination and case management services for patients, families, and the local community.

What you will do

• Assess patient needs throughout the discharge planning process.
• Develop safe and effective transition plans with patients, caregivers, community providers, and the healthcare team.
• Coordinate care and ensure communication among families, providers, and clinical staff.
• Provide resources and community referrals to support continuity of care.
• Conduct utilization reviews and follow Medicare conditions when required.

Requirements

• Bachelor’s degree in a healthcare-related field or equivalent experience.
• Current BSN or commitment to achieve the degree within the specified timeframe.
• 3–5 years of case management-related healthcare experience.
• Strong clinical experience and skills, clinical documentation ability, and experience using EMR systems.
• Work onsite in Manchester, New Hampshire.

Nice to have

• Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification.
