Clinical Pharmacist (Cardiometabolic)

Location: Remote, anywhere in the United States. Travel up to 10%. Applicants must already be authorized to work in the United States; visa sponsorship or transfer is not available.

What you will do

• Analyze medication utilization data to identify polypharmacy, adherence, prescribing, and high-cost drug trends.
• Develop medication optimization and cost-containment strategies for diabetes, hypertension, obesity, heart failure, asthma, depression, and other chronic conditions.
• Conduct comprehensive medication reviews, resolve drug therapy problems, optimize regimens, and track safety and clinical outcomes.
• Collaborate with physicians, nurses, care coordinators, and other multidisciplinary teams to operationalize virtual care interventions.
• Advise providers and educate patients on evidence-based therapies, medication use, side effects, and adherence.
• Document interventions in the EHR and report clinical, financial, adherence, and safety metrics supporting quality improvement and value-based contracts.

Requirements

• PharmD from an ACPE-accredited College of Pharmacy.
• Completion of an ASHP-accredited PGY-1 Pharmacy Residency; PGY-2 preferred.
• Active pharmacy license without state restrictions.
• At least 3 years of clinical pharmacy experience in ambulatory care, ideally in value-based care, a CIN, or an ACO.
• Experience with medication therapy management, evidence-based medicine, medication data analysis, analytics tools, cost-containment strategies, quality improvement, and EHR documentation.
• Current authorization to work in the United States without present or future visa sponsorship.

Nice to have

• Experience in telehealth, health plan, managed care pharmacy, population health, or quality improvement projects.
• Completion of a PGY-2 Specialty Pharmacy Residency.
