Clinical Care Coordinator (Healthcare Operations)

What you will do

• Coordinate individual member cases from referral through follow-up and clinical navigation.
• Complete insurance eligibility checks, prior authorisation requests, and specialist referrals within defined timelines.
• Track referral completion, close follow-up loops, and ensure no member falls through the gaps.
• Maintain accurate case documentation and clear communications with members, clinical teams, and external providers.
• Handle complex or sensitive situations with sound judgement and escalate them when required.

Requirements

• 2–4+ years of experience in patient navigation, referral coordination, care management, or healthcare operations.
• Strong understanding of US healthcare systems, including insurance eligibility, prior authorisation workflows, and specialist referrals.
• Experience with EHRs or care coordination platforms such as Epic, Athena, Elation, or Salesforce Health Cloud.
• Empathetic, professional handling of sensitive member situations and strong written and verbal communication.
• Excellent organisation and attention to detail across complex, multi-step workflows.
• Ability to work effectively in a fast-paced environment with changing priorities.

Nice to have

• Experience with multispecialty referral coordination or preventative healthcare models.
• Background in dermatology, cardiology, primary care, or other referral-heavy specialties.
• Familiarity with QA processes, documentation standards, or SOP frameworks.
• Experience in healthcare technology, digital health, or health-tech environments.
• Experience working within or alongside a clinical team in a high-volume care coordination setting.
