Family Nurse Practitioner (NP) OR Family Physician Assistant

Salary: $130,000–$140,000 per year

What you will do

• Conduct comprehensive clinical intake visits and provide longitudinal direct or wrap-around primary care.
• Manage chronic conditions such as hypertension, diabetes, and chronic kidney disease, while evaluating acute concerns and providing preventive and wellness care.
• Support behavioral health care, including depression, anxiety, alcohol use disorder, and opioid use disorder management.
• Conduct member outreach, provide timely follow-up, and close contractual and company-prioritized quality gaps.
• Complete accurate diagnosis and encounter documentation using HCC, ICD-10, and CPT coding.
• Collaborate with interdisciplinary care teams on care plans, case conferences, clinical programs, and member health goals.

Requirements

• 3+ years of patient-facing experience.
• Completion of an accredited Nurse Practitioner or Physician Assistant program with national certification.
• Unrestricted license to practice in the state where employed with Cityblock.
• Board certification, or board eligibility within one year of employment.
• Basic Life Support certification for healthcare providers.
• DEA licensure or willingness to acquire it.
