Community Engagement and Field Care Worker (Healthcare)

Location: Must reside within a 50-mile radius of Northern Kentucky. Remote field role with 60–80% travel within the Northern Kentucky area. Availability required Monday–Friday between 8am and 6pm Eastern Time, with occasional travel to hirify.global offices for training or meetings.

Salary: $58,700–$70,400 per year, plus eligibility for a bonus incentive plan.

What you will do

• Build visibility and credibility for hirify.global Healthy Horizons products and services in local communities.
• Engage members and community residents through advocacy programs and outreach activities.
• Conduct in-person consumer assessments and support the development of care plans.
• Develop strategies to scale community programs and strengthen connections among community members.
• Work with community health and social service resources to support underserved populations.

Requirements

• Must reside within a 50-mile radius of Northern Kentucky.
• At least two years of professional human services experience, or recent experience in the same role involving in-person consumer assessments or care plans.
• At least one year of community outreach, marketing, or sales experience.
• Community Health Worker Certification must be obtained within six months of hire.
• Excellent written and verbal communication skills and proficiency with Microsoft Office and electronic information systems.
• Valid state driver's license, personal vehicle liability insurance with 100,000/300,000/100,000 limits, TB screening, and a dedicated workspace meeting HIPAA privacy requirements.

Nice to have

• Bachelor's degree in Social Work or a health-related field.
• Spanish bilingual skills.
• Home health experience.
• Knowledge of community health, social service agencies, and local community resources.
