Outreach Representative (Healthcare)

Location: Remote, based in Pennsylvania, United States. Office-based position with a physical work site required; travel may occasionally be required, at 0%–25%.

What you will do

• Serve as a liaison between members, the health plan, and relevant state or federal regulators.
• Conduct telephone outreach for targeted preventive health initiatives and educate members about available services and their rights and responsibilities.
• Assess member needs, recruit and enroll eligible members in disease management programs, and refer cases for care management intervention.
• Coordinate access to preventive health services with members, providers, and community agencies.
• Support members through the grievance and appeals process and represent their interests internally and externally.
• Document member and provider contacts and contribute to measurable, innovative outreach programs for at-risk populations.

Requirements

• High school diploma or GED required.
• At least one year of experience in community outreach or customer service.
• Strong active listening, communication, empathy, patience, and problem-solving skills.
• Ability to work independently and collaboratively, prioritize multiple tasks, and exercise sound judgment.
• Awareness of healthcare access barriers, including transportation, childcare, financial challenges, and distrust of the healthcare system.
• Must be based in Pennsylvania, United States.

Nice to have

• Associate’s degree.
• Familiarity with healthcare or insurance.
• Experience working with Medicaid or Medicare populations.
