What you will do

• Conduct in-home clinical assessments to help patients and caregivers successfully begin services.
• Develop and update individualized plans of care based on patient needs and clinical conditions.
• Maintain ongoing relationships with patients through scheduled home visits and follow-ups.
• Coordinate care with physicians, internal clinical teams, and community resources.
• Educate and coach patients and caregivers on care plans and health and safety best practices.
• Ensure clinical documentation is accurate and meets regulatory requirements.

Requirements

• Graduate of an accredited nursing program.
• Active and unrestricted Registered Nurse license in the State of Colorado.
• Valid driver's license and active auto insurance for travel within the assigned geographic area.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently while collaborating with a clinical team.
• Comfort using common clinical and case management software.
