What you will do

• Perform person-centered in-home nursing assessments for members of all ages.
• Evaluate functional abilities and needs for ADL and IADL assistance.
• Coordinate care and ensure services are delivered according to state policies.
• Practice cultural humility and utilize evidence-based interventions.
• Travel within Missoula and Ravalli Counties to conduct field visits.
• Maintain accurate documentation and meet project timelines.

Requirements

• Current Montana Registered Nursing (RN) license required.
• Must reside in Missoula or Ravalli County, Montana.
• Ability to travel by personal car to locations throughout the assigned counties.
• Reliable home internet access.
• Experience with home health, hospice, or long-term care preferred.
