What you will do

• Manage completion of process steps not finished by the Care Coordinator, following established protocols.
• Facilitate the flow of evaluations, orders, and progress notes from providers to the Nurse Case Manager or adjuster.
• Review physician orders and patient assessments, communicate updates to providers and carriers, and ensure orders are obtained and followed.
• Locate providers and arrange additional services for current patients; obtain authorization for additional/continued services.
• Create purchase orders to authorize services and use tracking tools (authorization logs, Outlook Calendar/Tasks) to meet critical timelines.
• Coordinate quality assurance concerns, maintain up-to-date patient database documentation, and close files per procedure.

Requirements

• Registered Nursing degree (R.N. or L.P.N.) with at least 2 years of experience in an acute setting, home care, or front/back medical office.
• Strong verbal and written communication skills for professional coordination with providers and carriers.
• Excellent customer service skills and ability to resolve inquiries using company products/services knowledge.
• Ability to work independently and in a team environment while following established processes.
• Computer navigation and operation skills for electronic documentation and tracking.
• Work-from-home capability: set up computer equipment in a home office and perform primarily sedentary work.
