TL;DR

Housing Case Manager: Delivering community-based case management services to individuals experiencing mental illness, intellectual disabilities, or substance use disorders with an accent on housing support and resource coordination. Focus on conducting bio-psychosocial assessments, developing crisis coping mechanisms, and ensuring high-quality, recovery-oriented care within the Algoma community.

Location: Must be based in Algoma, Canada (Onsite role)

Salary: $32.17–$39.30 per hour

The Canadian Mental Health Association, Algoma Branch, provides accessible, recovery-oriented mental health and wellness services to the local community.

What you will do

  • Deliver case management and housing support to individuals who are homeless or at risk of homelessness.
  • Conduct bio-psychosocial assessments to identify client needs and support systems.
  • Coordinate and monitor the use of community resources and supportive housing rent supplement units.
  • Develop crisis plans and help clients build effective coping mechanisms.
  • Collaborate with internal teams and external partners to promote efficient service delivery.

Requirements

  • University degree in a Human Services related field.
  • Minimum three years of experience in mental health and addictions, specifically in community-based services.
  • Current registration with OCSWSSW or a regulated body under the Regulated Health Professions Act.
  • Mandatory certifications: First Aid/CPR, ASIST, and Non-Violent Crisis Intervention.
  • Ability to provide effective services in both English and French is preferred.
  • Must pass a Police Vulnerable Sector Check upon hire.
