What you will do

• Lead all Community Health Worker (CHW) operations, including workforce planning, deployment, productivity, and performance management.
• Establish consistent standards for home visits, community-based interventions, member engagement, and field documentation.
• Build management systems that support coaching, accountability, quality, and professional development across the CHW organization.
• Partner with Nurse Practitioners, RN Care Managers, and Behavioral Health Case Managers to ensure seamless integration within care teams.
• Monitor operational performance to identify opportunities for improving member engagement and retention.
• Support scaling the CHW function and maintaining operational excellence during expansion into new markets.

Requirements

• 8+ years of healthcare operations leadership experience.
• Experience leading Community Health Workers, care coordinators, patient navigators, or similar community-based workforces.
• Experience building and managing distributed field teams.
• Strong understanding of Medicaid populations and social determinants of health.
• Proven success in developing frontline leaders and improving operational performance.
• Must be authorized to work in the United States.

Nice to have

• Experience with ECM, Community Supports, CalAIM, or other Medicaid care management programs.
• Experience working with culturally diverse and historically underserved populations.
• Multi-state operational experience.
• Bilingual Spanish/English proficiency.
