Pediatric Advanced Practice Provider (Virtual Care): Deliver virtual-first and in-home clinical care for children with special health care needs with an accent on complex pediatric panel management, multidisciplinary care coordination, and continuity of care across enrollment, acute escalations, sick visits, and post-hospital follow-ups. Focus on reducing unnecessary medical visits and hospitalizations while partnering with families and an interdisciplinary team to increase safe days at home.
Location: Tennessee, United States (Remote) — must reside and be licensed in Louisiana or Tennessee. Travel up to 10%; schedule requirements: 4x10s, 8:00 am–6:00 pm (Monday–Friday) local time.
Salary: $115,000+ (plus annual bonus)

What you will do
- Manage a panel of complex pediatric patients by overseeing care plans, collaborating with PCPs and specialists, and coordinating comprehensive care.
- Provide wrap-around care in a multidisciplinary team (pediatricians, registered nurses, pharmacists, dietitians, social workers, and therapists) within a virtual-first model.
- Educate and support families to help them navigate their child’s health journey and improve long-term outcomes.
- Deliver high-quality virtual care including virtual enrollment visits, acute escalations, sick visits, and post-hospital follow-ups, plus in-home virtual assessments supported by the Mobile Integrated Care team.
- Ensure continuity of care by bridging care gaps, coordinating prescriptions, and streamlining referrals.
Requirements
- Active LA or TN licensure required (unrestricted NP or PA state license) and board certification.
- Completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C).
- Minimum 5 years of experience with a pediatric population; community-based health or complex care strongly preferred.
- Experience and comfort with children with complex medical needs.
- Ability to lead and guide a clinical team.
- Bilingual Spanish is strongly preferred.
