Location: Must be based in the USA and hold active licenses in AZ, CO, HI, and/or OR. Working hours: Monday-Friday 8:30am-5:00pm MST or PST.

Salary: $120,000–$150,000 base + performance bonus (Total: $130,000–$170,000).

What you will do

• Deliver exceptional clinical care to complex patients through video telehealth visits.
• Manage clinical trajectories, including symptom and chronic disease management.
• Collaborate with an interdisciplinary team of physicians, case managers, and social workers.
• Perform comprehensive assessments, post-acute transition support, and medication reviews.
• Coordinate care plans with PCPs, specialists, and patient support systems.
• Maintain accurate medical records in compliance with legal and ethical standards.

Requirements

• Active and clear RN and APRN or Physician Assistant license in AZ, CO, HI, and/or OR.
• 3-5+ years of experience as an APP in primary care, geriatrics, or palliative care.
• Proficiency in English and Spanish is highly preferred.
• Active BLS certification.
• Experience with telehealth technology and electronic health records (EHR).
• Must be available to work Monday-Friday 8:30am-5:00pm MST or PST.
