What you will do

• Conduct in-home assessments and functional evaluations for clients.
• Deliver personalized, evidence-based recommendations for home modifications.
• Educate clients and caregivers on fall prevention and safe mobility techniques.
• Engage clients and families in collaborative care planning.
• Collaborate with the Director of Clinical Operations to support process improvements.
• Maintain accurate and timely documentation via the electronic system.

Requirements

• Degree in Occupational Therapy from an accredited program.
• Active, unencumbered OT license.
• 2+ years of clinical experience.
• Reliable transportation and valid driver’s license.
• Proficiency with technology and digital documentation.
• Ability to work independently while collaborating with a remote team.

Nice to have

• Experience in home health, aging in place, or disability services.
• Prior experience with home modification projects.
