What you will do

• Engage and support individuals with mental health and substance use disorders through an omnichannel virtual approach.
• Identify care goals, screen for unmet needs, and address barriers to definitive behavioral health care.
• Navigate individuals to appropriate primary and behavioral health services, including scheduling and follow-ups.
• Collaborate with the clinical team through case reviews to ensure effective care transitions.
• Manage the end-to-end navigation funnel and provide clinical supervision to other care team members.
• Develop and refine workflows and policies to improve the efficiency of the navigator team.

Requirements

• Active Registered Nurse (RN) license or Clinical Social Work license required.
• Must be based in the United States.
• 3+ years of experience in care coordination or case management within a health plan or at-risk provider setting.
• Experience in a high-volume, metric-driven contact center environment.
• Strong technical proficiency with CRM systems and omnichannel communication tools.
• Demonstrated experience supporting individuals with complex needs, including chronic conditions or homelessness.
