What you will do

• Conduct clinical outreach and educate families about Virtual Intensive Outpatient Program (VIOP) services.
• Screen and assess prospective members for program eligibility and clinical appropriateness.
• Guide families through the admissions process, including insurance and financial expectations.
• Coordinate care schedules across multidisciplinary teams, including therapy and psychiatric services.
• Serve as the primary point of contact for members and families, providing proactive case management.
• Maintain accurate documentation in the electronic health record (EHR) and assist with discharge planning.

Requirements

• Master’s degree in Social Work (MSW) and Associate Registration (or state equivalent).
• Full-time resident of the United States.
• Experience working with pediatric and adolescent populations and their families.
• Proficiency with computer and live video tools (e.g., Zoom, Google Meets).
• Skilled in case management, care coordination, and collaborative care delivery.
• Ability to work a set schedule of Monday - Friday, 11:00am - 8:00pm PST.
