What you will do

• Implement and manage Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs directly within clinics.
• Engage patients in person to review care plans, answer questions, and monitor health goals.
• Act as a primary liaison between patients, clinical staff, and the internal care management team.
• Handle program enrollments, patient follow-ups, and ongoing care monitoring.
• Prepare relevant health data for patient visits to support physicians and clinical staff.
• Collaborate with the care management team to identify and implement process improvements.

Requirements

• Must be located near the Detroit, MI area.
• Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification.
• 1+ years of experience in a clinical setting, preferably in care or chronic disease management.
• Proficiency in medical terminology and clinical processes.
• Willingness to travel to various clinic locations within the Detroit, MI area.
• Strong organizational and communication skills for fast-paced environments.
