Salary: $74,000 - $150,000 / year

What you will do
- Work directly with patients to manage chronic conditions through personalized care plans.
- Conduct comprehensive medication reviews and perform medication therapy management (MTM) for complex regimens.
- Identify and resolve drug therapy issues and therapy gaps to optimize care and health outcomes.
- Develop and deliver educational materials tailored to the local population and support medication adherence.
- Collaborate with interdisciplinary teams to iterate on disease management processes.
- Support on-call rotations for urgent pharmacy needs and act as a clinical subject matter expert.
Requirements
- Residency: Must currently reside in the State of Hawaiʻi.
- Licensure: Active Pharmacist License in the State of Hawaiʻi (in good standing) at time of application.
- 3+ years of clinical experience in direct patient care.
- Ability to communicate complex medical information with a “talk story” warmth that builds trust with local patients.
Nice to have
- Experience with collaborative pharmacy practice agreements.
- Board certification or specialized experience in complex geriatric care (Kupuna care).
- Multilingual skills (e.g., Tagalog, Ilocano, Cantonese, or Hawaiian).
