What you will do

• Oversee care and outcomes for a patient cohort in collaboration with nurse practitioners.
• Conduct in-home and telehealth visits for Medicare Advantage members using Clover Assistant and EMR.
• Coordinate with interdisciplinary teams to create personalized care plans and close gaps in care.
• Provide clinical leadership and mentorship to nurse practitioners to foster professional growth.
• Lead quality improvement initiatives and conduct assurance reviews to optimize care processes.

Requirements

• MD or DO degree with board certification in Internal, Family, or Geriatric Medicine.
• Valid license in New Jersey (or willingness to obtain), including DEA and CDS registrations.
• Minimum of five years of advanced clinical practice experience.
• Proven expertise in chronic disease management for vulnerable senior populations.
• Experience in home care (preferred).
