Location: Must be based in Delaware. This is a community-based role requiring frequent travel within the assigned territory in DE to member homes and nursing facilities.

What you will do
- Conduct regular face-to-face needs assessments in member homes and nursing facilities.
- Coordinate care across physical, behavioral, and long-term service delivery systems.
- Authorize and manage long-term services and supports (LTSS) based on comprehensive assessments.
- Facilitate care transitions between hospitals, nursing facilities, and home settings.
- Develop and evaluate individualized care plans to meet member goals and overcome obstacles.
- Collaborate with integrated care teams and providers to ensure safe, least-restrictive care environments.
Requirements
- Must be based in Delaware with reliable transportation for frequent travel.
- Bachelor's degree in Social Work/Health/Human Services with 3 years of experience OR Master's degree with 1 year of experience.
- Alternatively, current RN or LPN licensure with 2 years of relevant experience.
- Alternatively, high school degree with 3 years of case management experience for aged/disabled populations.
- Proficiency in PC-based word processing and electronic documentation systems.
- Ability to maintain a dedicated home workspace and comply with telecommuter policies.
Nice to have
- Certified Case Manager (CCM) or relevant social work licensure (LBSW, LMSW, LCSW).
- Experience with HIV/AIDS, behavioral health, or developmental disabilities populations.
- Prior experience with Medicare, Medicaid, or managed care environments.
