Location: Must be based in or be able to relocate to Southern New Jersey

Salary: $250,000 - $280,000 USD

What you will do

• Oversee care and outcomes for a patient cohort, managing cases in collaboration with nurse practitioners.
• Conduct in-home and telehealth visits for Medicare Advantage members using Clover Assistant and EMR.
• Coordinate with interdisciplinary care teams to create personalized care plans and close gaps in care.
• Provide clinical leadership and mentorship to nurse practitioners to foster professional growth.
• Lead quality improvement initiatives and conduct assurance reviews to advance clinical excellence.
• Collaborate with the Medical Director and Practice Manager to optimize care processes and performance.

Requirements

• MD or DO degree with board certification in Internal Medicine, Family Medicine, or Geriatric Medicine.
• Must be licensed in New Jersey (or willing to become) with DEA and CDS registrations.
• Minimum of five years of advanced clinical practice experience.
• Proven expertise in chronic disease management for vulnerable senior populations.
• Strong alignment with using technology to drive high-quality clinical care.
