What you will do

• Deliver exceptional virtual care via AI platform and video telehealth to complex patients across geographies.
• Clinically manage high-risk geriatric patients focusing on conditions like polypharmacy, falls, and dementia using the 5M framework.
• Lead interdisciplinary teams by monitoring outcomes, trending metrics, and ensuring performance standards.
• Assess and improve clinical models, AI applications for better outcomes, and act as a change agent.
• Engage patients, caregivers, PCPs, specialists, and internal teams for cohesive care experiences.
• Use electronic health system (Orinoco) for visits and provide feedback for improvements.

Requirements

• Board Certified in Geriatrics, Primary Care, Internal Medicine, or Family Medicine with at least 2 years clinical experience.
• Experience providing clinical care in collaboration with interdisciplinary teams.
• Must be eligible to work in the USA (inferred from remote USA and US benefits).

Nice to have

• Experience with complex, frail, or vulnerable populations, including SNF settings.
• Virtual care experience.
• Experience serving diverse patient populations and focus on care quality measurement.
