Complex Care Physician (AI-Enabled Care)

Salary: $210,000–$315,000 per year

What you will do

• Deliver direct virtual care to a cohort of high-risk and complex patients through video telehealth and an AI-enabled care platform.
• Manage complex geriatric conditions, including polypharmacy, falls, and dementia, using the 5M framework.
• Lead interdisciplinary teams and monitor clinical outcomes, performance trends, and care-quality metrics.
• Improve the clinical model and evaluate how AI can enhance patient outcomes.
• Coordinate care with patients, caregivers, primary care physicians, specialists, pharmacy, disease management, education, social work, and palliative teams.
• Use the Orinoco electronic health information system and provide feedback to improve its interface.

Requirements

• Board certification as a geriatrician, primary care, internal medicine, or family medicine physician.
• At least two years of clinical experience.
• Experience providing clinical care in collaboration with interdisciplinary teams.
• Ability to work remotely from the United States.

• Experience with complex, frail, or vulnerable populations, including in a skilled nursing facility setting.
• Experience with virtual care.
