What you will do

• Lead the Care Coordination team, overseeing hiring, coaching, and performance metrics for member intake and onboarding.
• Operationalize healthcare contracts by translating payer and provider requirements into clear SOPs and eligibility verification logic.
• Coordinate cross-functionally with Culinary, Nutrition, and Logistics teams to align MTM menus and delivery parameters.
• Partner with Finance and Revenue Operations to identify operational errors contributing to claim denials and improve reimbursement cycles.
• Ensure all communication and operational workflows remain strictly HIPAA-compliant.
• Build scalable infrastructure, including operational playbooks and training materials, to support program growth.

Requirements

• 3-5 years of experience in Healthcare Operations, Care Coordination, or Healthcare Program Management.
• Experience working with healthcare payers, providers, or value-based care programs.
• Proven experience managing frontline operational teams.
• Strong understanding of healthcare workflows and HIPAA compliance requirements.
• Ability to translate contractual or clinical requirements into actionable operational processes.
• Familiarity with Zendesk, Salesforce, or healthcare CRM platforms.
