What you will do

• Supervise the Promotora de Salud (PdS) team through regular 1:1 check-ins and performance reviews.
• Monitor team skills and implement tools to document performance and progress on program goals.
• Ensure timely documentation of activities and implement quality assurance measures.
• Coordinate community events and act as a stakeholder in program model changes.
• Facilitate case conferencing and provide on-call consultation for the team.
• Identify and escalate member feedback to improve healthcare products and services.

Requirements

• Bachelor’s Degree or equivalent experience in the social services field.
• Must reside in Merced, CA.
• 3+ years of experience in a healthcare setting with knowledge of Medicaid/Medicare.
• 2+ years of experience as a CHW/PdS and 2+ years of experience supervising frontline staff.
• Fluency in English and Spanish (verbal and written).
• Ability to follow HIPAA rules and maintain strict confidentiality of sensitive information.
