Nurse Case Manager Care Coordination Part Time Days

Location: 25 N. Winfield Rd., Winfield, Illinois, United States. Part-time day schedule, 0.7 FTE, 8:00–16:40; one weekend every seven weeks and one holiday per year.

Salary: USD 40.50–64.80 per hour

What you will do

• Coordinate patient care from admission through discharge, ensuring appropriate level and site of care.
• Assess, plan, implement, and evaluate care plans with patients, families, physicians, and multidisciplinary teams.
• Perform utilization management for inpatient and observation patients using approved criteria and Milliman guidelines.
• Coordinate discharge planning, referrals, post-acute services, bed placement, and safe handoffs to continuing-care providers.
• Identify barriers to patient flow, resolve level-of-care concerns, and escalate outlier cases to physician advisors and management.
• Provide clinical reviews to third-party payers and support accurate medical-necessity determinations.

Requirements

• Bachelor of Science in Nursing is required for hires after January 1, 2011.
• Registered Nurse license in the state of Illinois required.
• At least one year of case management experience in an acute hospital setting.
• Ability to collaborate with physicians, nursing staff, patients, families, payers, and post-acute care providers.
• Background check required for newly hired team members.

Nice to have

• Two years of utilization review experience.
• Case management certification.
• Prior case management or insurance-setting experience.
• Bilingual communication skills.
