Location: 25 N. Winfield Road, Winfield, Illinois, United States; home-health patient care is performed in person.

Salary: $31.71–$41.23 per hour

What you will do

• Provide direct physical therapy assistance and adapt treatments to individual patient needs.
• Use treatment modalities, equipment, and protocols under the supervision of a licensed physical therapist.
• Document patient responses, treatment performance, assessments, and discharge-related needs in the electronic medical record.
• Collaborate with physical therapists and healthcare professionals to support safety, functional progress, and treatment outcomes.
• Educate patients and provide materials appropriate to their understanding and motivation.
• Participate in department improvement initiatives, committees, education, mentoring, and clinical training activities.

Requirements

• Associate degree from a Physical Therapist Assistant program.
• Licensed Physical Therapist Assistant in the state of Illinois.
• BLS certification from the American Heart Association upon hire.
• Ability to complete documentation and maintain department productivity standards.
• Ability to flex the schedule for patient-care needs, staff meetings, and department meetings.
