Outpatient Physical Therapist-Full-time Days

Location: 2111 Midlands Court, Sycamore, Illinois, United States; full-time, rotating shift.

Salary: $40.79–$55.07 per hour.

What you will do

• Provide direct outpatient physical therapy and manage patient care from initial evaluation through discharge.
• Select appropriate evaluations, treatment modalities, equipment, and protocols; perform re-evaluations and adjust treatment plans.
• Document evaluations, progress notes, daily treatment responses, and discharge summaries in the electronic medical record.
• Collaborate with physical therapist assistants, physicians, nurses, case managers, patients, and families.
• Provide patient education and contribute to committees, process improvement, service development, and cost-efficiency initiatives.
• Support clinical education through student supervision, mentoring, in-services, journal clubs, and new-hire orientation.

Requirements

• Bachelor’s degree in Physical Therapy.
• Current physical therapist license in the State of Illinois.
• Basic Life Support certification from the American Heart Association upon hire.
• Ability to meet department productivity, documentation, collaboration, and patient-care standards.
• Willingness to adjust schedules or assignments to meet patient-care and department needs.

Nice to have

• Master’s or Doctoral degree in Physical Therapy.
