RN, Senior LTSS Service Care Manager (Pediatric Care)

Location: Remote in Texas, with required member assessment visits near Katy, Richmond, Rosenberg, and Sugar Land, TX. Standard schedule is Monday–Friday, 8:00 AM–5:00 PM CST, with flexibility for pediatric members' school schedules.

Salary: $36.21–$65.09 per hour

What you will do

• Assess complex and high-acuity pediatric members and recommend person-centered care and service plans.
• Develop, monitor, and update long-term care plans in collaboration with members, caregivers, providers, specialists, and community resources.
• Coordinate healthcare services, treatments, authorizations, referrals, and community resources including transportation, housing, meals, foster care, and activities of daily living.
• Monitor clinical status, complications, care gaps, waiver eligibility, and potential needs for higher levels of care.
• Educate members and families on disease processes, care options, provider instructions, benefits, and available services.
• Conduct required home or site visits, document care management activities, support regulatory compliance, and advocate for members.

Requirements

• Graduate of an accredited school of nursing.
• Registered Nurse state licensure or compact state licensure; Nurse Practitioner licensure may also qualify.
• 4–6 years of related experience, preferably in pediatric acute care, pediatric home health, or pediatric private-duty nursing.
• Bachelor of Nursing degree preferred.
• Texas Department of Family & Protective Services background check required.
• Travel to conduct member assessments is required; service delivery covers areas near Katy, Richmond, Rosenberg, and Sugar Land, Texas.

Nice to have

• Experience in home health, hospice, long-term care, or managed care.
• Resource Utilization Group certification; required for Superior and must be obtained within 90 days of hire.
• Experience precepting, coaching, or onboarding clinical staff.
