LTSS Service Care Coordinator

Location: Granite City, Illinois; candidates should reside within 30 miles of Alton/Wood River in Madison County, Illinois. The role combines remote work with approximately 50% local travel for in-home member visits. Training is Monday-Friday, 8:00 AM-5:00 PM CT; the regular schedule is Monday-Friday, 8:00 AM-4:30 PM CT.

Salary: $22.94-$38.79 per hour

What you will do

• Evaluate member needs, available resources, and potential care outcomes.
• Develop and monitor long-term care plans and service plans for members receiving LTSS.
• Coordinate services among members, families, caregivers, healthcare providers, specialists, and community resources.
• Conduct on-site and in-home visits, assess member needs, and support access to services.
• Educate members and caregivers about procedures, provider instructions, service options, referrals, and healthcare benefits.
• Document care management activities, monitor outcomes, and provide recommendations to improve quality and cost-effectiveness.

Requirements

• Bachelor's degree and at least one year of related experience.
• For the Illinois Physically Disabled/Elderly plan, candidates must be an Illinois-licensed RN, hold a qualifying bachelor's or master's degree in a human services-related field, or be an LPN with one year of experience conducting comprehensive assessments and providing formal services for elderly people.
• Experience in case management, advocacy, or home visits is strongly preferred.
• Ability to perform approximately 50% local travel and in-home member visits is required.
