Nurse Practitioner (1099 / PRN) – In Home Assessments

Location: Searcy, Arkansas, United States; on-site field work within a 55-mile radius of the assigned county

Pay: approximately $100 per completed assessment plus paid mileage

What you will do

• Conduct in-home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, physical exams, vital-sign checks, medication reviews, and screenings.
• Complete Hemoglobin A1C, spirometry, KED, DRE, and FIT screenings.
• Educate members about preventive care and chronic disease management.
• Document visits in the EMR to support value-based care and risk adjustment.
• Travel daily within the assigned county area to complete scheduled visits.

Requirements

• Active, unencumbered Nurse Practitioner license in the applicable state.
• Three or more years of patient care experience preferred.
• Comfort with EMR systems, independent work, and field-based patient visits.
• In-home assessment or risk-adjustment experience preferred.
• Maintain approximately 30 hours of monthly availability on an ongoing basis and honor confirmed assignments.

Nice to have

• Previous in-home assessment experience.
• Previous risk-adjustment experience.
• Willingness to obtain additional licensure with company support.
