Registered Nurse Care Manager (Healthcare)

Location: New Richmond, Wisconsin, with service coverage across northern St Croix and Pierce counties. The role is home-based with approximately 40% travel for member visits and occasional travel to hirify.global offices for training or meetings. Working hours are Monday through Friday, 8:00 am–4:30 pm CST.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess members’ health and safety needs and develop comprehensive Member Care Plans.
• Conduct regular health assessments and maintain accurate member documentation within DHS contract timeframes.
• Coordinate care with acute-care and primary-care providers.
• Educate members about disease processes, nutrition, exercise, lifestyle changes, and related risks.
• Complete monthly phone contacts and travel to members’ settings for in-person visits.

Requirements

• Associate degree in nursing.
• Active Registered Nurse license in Wisconsin in good standing.
• Ability to reside within 45 minutes of the assigned coverage area in St Croix/Pierce counties.
• Valid state driver’s license and personal vehicle liability insurance with minimum limits of 100/300/100.
• Ability to work from a dedicated home workspace with internet speeds of at least 25 Mbps download and 10 Mbps upload.
• Ability to complete required TB screening for this patient-facing role.

Nice to have

• At least one year of care management experience.
• Previous experience in care management.
