Community Health Worker

TL;DR

Community Health Worker: Serve as a liaison between members, providers, and community resources with an accent on outreach, health education, and care navigation. Focus on conducting health risk assessments, coordinating preventive health services, and partnering with community centers to close care gaps and improve wellness.

Location: Remote (must reside within 2 hours of the Hampshire county area: Morgan, Berkley, Jefferson, and Monongalia)

What you will do

• Initiate face-to-face outreach for targeted preventive health, maternity, care transitions, and chronic condition initiatives.
• Educate members, assess opportunities for care management intervention, and make appropriate referrals (including behavioral health as needed).
• Assist members with completing a Health Risk Assessment and help address barriers such as knowledge gaps, transportation, and financial obstacles.
• Coordinate access to preventive health services by contacting members, providers, and community agencies.
• Develop rapport with members and community centers; represent the organization at community events.
• Collaborate with human services providers and state agencies to identify and outreach to members in need of services.

Requirements

• Residency requirement: must reside within 2 hours of the Hampshire county area (Morgan, Berkley, Jefferson, and Monongalia)
• 1 year of experience in health care related customer service or marketing that demonstrates ability to positively impact member engagement in condition and case management programs
• Basic medical terminology background
• Knowledge of customer service principles (needs assessment and meeting quality standards)
• Strong written and oral communication skills; ability to work well with peers, supervisors, and customers
• Demonstrated cultural competency and sensitivity toward the Medicaid population, including working with members with limited English proficiency
