What you will do

• Conduct virtual adult and pediatric wellness visits and comprehensive health assessments.
• Perform health risk assessments, medication reviews, and cognitive screenings.
• Collaborate with in-home clinical team members (Sprinters) to evaluate vital signs and arrange blood draws.
• Provide care coordination, including referrals to specialists and community resources.
• Manage patient documentation and preventive care interventions.

Requirements

• Board Certified as a Family Nurse Practitioner.
• Active Family Nurse Practitioner license in Louisiana.
• Proficiency in using laptops, mobile devices, and HIPAA-secure messaging apps.
• Strong written and verbal communication skills.
• Ability to pass a national background check and clinical license search.
