What you will do

• Screen members for Diabetes Clinic eligibility and assess their clinical needs.
• Provide ongoing education on disease processes, medications, glucose monitoring, and lifestyle counseling.
• Collaborate with nurse practitioners, social workers, and pharmacists to develop and optimize care plans.
• Utilize motivational interviewing to influence positive behavioral changes and help members achieve SMART goals.
• Maintain accurate documentation of all member and provider interactions.
• Connect members with internal resources to troubleshoot clinical issues and address gaps in care.

Requirements

• Must be located within one of the contiguous 50 states
• Active unrestricted Compact Registered Nurse license or unrestricted Registered Dietitian license.
• Active Certified Diabetes Care and Education Specialist (CDCES) certification.
• At least 2 years of experience clinically managing diabetes.
• Ability to obtain additional state licenses as required by operations.

Nice to have

• Remote experience managing diabetes.
• Experience working with diabetes technology such as CGMs and insulin pumps.
