Location: Remote or hybrid (from Nashville or New York City offices)

Salary: $246,500-$290,000 (base salary if filled within New York City)

What you will do

  • Lead the evolution of the transitional case management program, including clinical vision, program design/iteration, protocols, escalation pathways, staff training, and KPI tracking.
  • Collaborate with technology and product teams to ensure transitional care tools reflect current clinical evidence and meet member needs.
  • Serve as a clinical resource for the frontline care team through case conferences, education, guidance, and support for complex discharge planning and post-acute needs.
  • Partner with Oncology APPs, Nurse Navigators, and Community Health Workers on challenging cases and contribute to ongoing care team education and feedback.
  • Lead and contribute to special projects and strategic initiatives, including protocol development and efforts to reduce avoidable readmissions and emergency utilization.
  • Represent hirify.global in external meetings with payers, health systems, and post-acute partners and support clinical content for external questions.

Requirements

  • At least 3 years of clinical experience caring for oncology patients after fellowship training.
  • Board certified in medical oncology.
  • Unrestricted medical license and willingness to obtain additional state licenses as needed.
  • Experience with transitional care management programs, care transition protocols, or reducing avoidable readmissions and emergency utilization in an oncology or complex care population (exceptional).
  • Comfort using and learning new technologies; experience with video chatting, Google Suite, Slack, and electronic health records.
  • Strong organization, communication, and ability to prioritize urgent member needs under pressure.
