What you will do

• Perform high-volume outbound telephonic outreach to enroll children in care and re-engage caregivers.
• Handle inbound calls from caregivers, health plan service coordinators, and hospital discharge planners.
• Conduct research on patient healthcare needs and utilization to identify barriers to enrollment.
• Collaborate with RNs, Community Health Workers, and Provider Engagement Specialists.
• Perform embedded on-site outreach at PCP offices and residential facilities when appropriate.
• Develop knowledge of the Supplemental Security Income (SSI) program to conduct targeted outreach.

Requirements

• 2+ years of experience in healthcare care coordination, community health, patient engagement, or sales.
• Proven success working in a centralized contact center environment.
• Bilingual Spanish required.
• Must be based in Texas or Florida.
• Ability to travel 10%-15% to provider offices and access to reliable transportation.
• Reliable internet access.
