Location: Remote within USA (specifically South Carolina, Georgia, or Oregon). Role is predominantly virtual but requires the ability to perform a small volume of home-based visits.

What you will do

• Conduct telehealth video visits and a small volume of in-home visits to members.
• Perform comprehensive assessment visits (CAVs) including diagnosis, medication review, and quality of care (STARS/HEDIS) interventions.
• Administer diagnostic tests, including mobile screenings and fingerstick tests.
• Coordinate collaborative care plans with PCPs, specialists, pharmacy, clinical nursing, and social work.
• Utilize and provide feedback to improve the proprietary electronic health information system.
• Provide infrequent weekend on-call support to clinical nurses for member triage.

Requirements

• Active and clear RN and APRN license in the state of hire (SC, GA, or OR).
• Active BLS certification required at time of hire.
• APRN with 3 or more years of experience in outpatient clinical practice.
• Experience in primary care, family medicine, geriatrics, and/or palliative care.
• Experience performing Medicare annual wellness visits and utilizing telehealth platforms.
• Willingness to perform home visits and obtain additional state licensures as requested.

Nice to have

• Prior experience in home-based clinical practice.
• Understanding of managed care and assessment of STARS/HEDIS measures.
• Bilingual proficiency in Spanish or Haitian Creole.
