Housing Navigator/Case Manager (Healthcare)

Compensation: $29.70 - $31.00 per hour

What you will do

• Conduct housing assessments and develop individualized support plans for members.
• Search for housing options and build strategic relationships with local landlords and property managers.
• Assist members with application completion, documentation gathering, and securing rent subsidies (Section 8, state/local programs).
• Provide advocacy for benefits, including SSI eligibility and application processes.
• Conduct health and safety home visits and habitability inspections.
• Implement early intervention services to prevent eviction and resolve lease violations.

Requirements

• Bilingual English/Spanish proficiency is required.
• Must be authorized to work in the United States.
• 2+ years of experience in housing navigation or case management.
• Knowledge of local housing markets, regulations, and tenant rights.
• Experience working with vulnerable populations.

Nice to have

• Bachelor's degree in Social Work, Health Services, or a related field.
• Experience with Housing Management Information Systems (HMIS).
• Knowledge of trauma-informed care practices.
• Previous experience in healthcare or managed care settings.
