What you will do

• Build trusted relationships with members and act as the primary point of contact throughout their healthcare journey.
• Assess medical, psychosocial, and functional needs and develop, implement, review, and adapt personalised care plans.
• Coordinate hospitals, specialists, physicians, caregivers, support services, and other healthcare providers.
• Monitor progress, support safe transitions of care, and identify emerging risks or care needs.
• Deliver specialised clinical programmes, including Case Management, Chronic Condition Management, Lifestyle Coaching, and Pre-Assignment Assessments.
• Maintain accurate case records, support quality and continuity of care, and promote clinically appropriate and cost-effective healthcare.

Requirements

• Bachelor's degree in Nursing.
• 3–5 years of experience in Case Management, Clinical Management, or a related healthcare setting.
• Fluent English is required.
• Strong clinical assessment, care coordination, organisation, prioritisation, communication, and problem-solving skills.
• Ability to advocate for members while complying with clinical, contractual, legal, and organisational requirements.
• Proficiency with Microsoft Office applications and healthcare management systems.

Nice to have

• Experience in an international environment.
• Proficiency in French, Portuguese, or German.
