RN Field Care Manager Nurse (Healthcare)

Location: Must reside in Michigan, specifically Wayne or Macomb Counties, and travel 75–90% within the assigned area for in-home visits. The role is based out of a home office, with occasional travel to hirify.global offices for training or meetings. Work hours are Monday through Friday, 8:30 AM–5:00 PM Eastern Time.

Salary: $71,100–$97,800 per year, plus eligibility for a performance-based bonus.

What you will do

• Manage a caseload and complete clinical assessments in members’ homes, community settings, and by phone.
• Provide clinical guidance to members with complex medical needs and coordinate appropriate services.
• Develop and update individualized care plans with members, caregivers, primary care providers, and the interdisciplinary care team.
• Assess medical complexity, complete attestations, and perform clinical oversight activities.
• Address health, housing, social, and educational barriers by connecting members with community resources.
• Coordinate as the primary point of contact for the interdisciplinary care team and external service providers.

Requirements

• Residence in Michigan, specifically Wayne or Macomb Counties, with the ability to travel to members’ homes and community settings.
• Active, unrestricted Michigan RN or APRN license, including Nurse Practitioner or Clinical Nurse Specialist credentials.
• At least two years of healthcare and/or case management experience.
• Experience working with adults and in disease management.
• Knowledge of community health, social service agencies, and local resources.
• Computer proficiency with electronic medical records, Microsoft Word, Outlook, and Excel, plus strong keyboard and web navigation skills.

Nice to have

• BSN and experience with in-home assessment, care coordination, health promotion, coaching, or wellness.
• Experience with Medicaid Long Term Care or managed care.
• Case Management or Motivational Interviewing certification.
• Bilingual Spanish, Arabic, or Chaldean Neo-Aramaic skills.
