Salary: $80,000 - $102,000 per year

What you will do

• Serve as the lead subject matter expert on chronic disease prevention and management (e.g., diabetes, cardiovascular disease, vaccinations).
• Collaborate with state healthcare partners and systems to drive measurable improvements in patient outcomes.
• Analyze state- and provider-level quality data and provide technical assistance to facilities.
• Facilitate learning collaboratives to implement evidence-based practices.
• Act as the primary point of contact for clients regarding contract management, communication, and dispute resolution.

Requirements

• Internal candidates only.
• Minimum 8 years of healthcare experience (preference in prevention and chronic disease).
• 1 year of previous supervisory and/or leadership experience preferred.
• Ability to travel between 15% and 60%.
• Excellent written and verbal communication and organizational skills.
