What you will do

• Deliver 1:1 coaching sessions at local health or community venues.
• Guide participants through a structured 12-month behaviour change journey toward Type 2 diabetes remission.
• Build rapport and provide tailored support based on individual participant needs.
• Maintain accurate clinical records and track patient outcomes using digital systems.
• Collaborate with the Momenta Newcastle delivery team to ensure programme quality.

Requirements

• Must be registered as self-employed in the UK.
• Must hold relevant professional insurance for delivering health coaching.
• Relevant qualification: either a degree in a health-related subject or a UK health coaching certification.
• Experience with health interventions or coaching.
• Proficiency in using digital systems to handle patient data.
• Must be willing and able to commute to clinics in the Somerset and Wiltshire regions.
