What you will do

• Provide relationship-based primary care while practicing at the top of the nursing license.
• Deliver preventive care, wellness services, chronic disease management, urgent visits, and patient education.
• Build trusted, lasting relationships with patients and clients.
• Work autonomously in a value-based care model focused on health outcomes rather than patient volume.
• Use clinical resources such as UpToDate and RubiconMD to support patient care.

Requirements

• Active nurse practitioner license and current ANCC or AANP board certification.
• Independent practice provider status preferred where permitted by state regulations.
• Federal DEA number preferred and potentially required for full prescribing authority.
• CPR/BLS certification required by the start date.
• Independent family practice experience in routine wellness care, chronic care management, and urgent visits preferred.
