What you will do
- Provide eye and vision care, along with related general health and well-being services.
- Establish practice days and hours of operation.
- Control the patient schedule and set professional fee schedules.
- Operate independently as a lease holder inside the Sam’s Club location.
- Optionally use equipment, instrumentation, administrative support, or Opt-Tech support services for fair market value where permitted by state law.
Requirements
- Doctor of Optometry (OD) from an accredited optometry school and a valid state optometry license, or an Ophthalmologist (MD or DO) degree from an accredited school and a valid state ophthalmology license.
- General Liability Certificate.
- Professional Liability Insurance Certificate.
- W-9.
- This is an independent lease agreement, not employment with Walmart Inc. or its subsidiaries.
