Registered Nurse - Diabetes and Endocrinology (Diabetes Care/Endocrinology): Delivering ambulatory nursing care and chronic disease management for patients with diabetes and endocrine conditions with an accent on patient education, individualized care plans, and evidence-based clinical support. Focus on insulin and injectable therapy education, continuous glucose monitor and insulin pump support, telehealth triage, medication management, and accurate EMR documentation.
Location: Nashua, New Hampshire, United States. The role includes in-office care, telehealth services, and 40 hours per week in 10-hour shifts, with Wednesdays off.

What you will do
- Deliver comprehensive ambulatory nursing care focused on chronic disease management.
- Support diabetes care, including blood glucose monitoring, medication adherence, nutrition, weight management, and lifestyle changes.
- Educate patients on insulin administration, injectable therapies, continuous glucose monitors, and insulin pumps.
- Develop and implement individualized endocrine care plans with endocrinologists, advanced practice providers, and care teams.
- Administer medications and vaccines, perform point-of-care testing, and monitor lab results and treatment adjustments.
- Provide telephone triage and telehealth support, document care in the EMR, assist with pre-visit planning, and mentor clinical staff.
Requirements
- Associate of Science in Nursing required; Bachelor of Science in Nursing preferred.
- Current Registered Nurse license in New Hampshire, an NLC state, or Massachusetts, and BLS certification required.
- At least one year of professional nursing experience preferred; ambulatory care experience is a plus.
- Knowledge of nursing principles, EMR systems, and patient-centered care models.
- Strong communication, time management, adaptability, teamwork, compassion, and attention to detail.
