Location: Tufts Medical Center; the role includes working with other entities for heart failure transition-of-care and clinic coverage.

What you will do
- Provide advanced nursing care for an assigned caseload of heart failure and cardiology patients in collaboration with physicians and multidisciplinary staff.
- Conduct comprehensive medical histories and physical examinations, assess patient health, and diagnose care needs.
- Order and interpret laboratory, radiological, and screening tests, document progress, and communicate findings to physicians and patients.
- Develop and implement individualized care plans, perform procedures, prescribe and monitor medications, and arrange referrals and follow-up care.
- Provide patient and family education, discharge planning, disease-prevention counseling, and psycho-emotional support.
- Document encounters in the EMR, participate in performance improvement and staff meetings, and support team development as a nursing leader.
Requirements
- Master’s degree in Nursing.
- Advanced Practice Registered Nurse (APRN) license.
- Basic Life Support (BLS) certification.
- Ability to assess and care for patients across different age groups.
- Strong interpersonal, analytical, and patient-management skills.
- Ability to work in a clinical environment involving prolonged standing and walking, patient lifting and turning, infectious disease exposure, blood-borne pathogens, and clinical procedures.
